Complete Life Care Management Assessment of physical, cognitive,psychological, social,and legal needs by an experienced Certified Care Manager.
Care Manager will meet with clients to explore their vision of the future and translate theirgoals into targeted and realistic options.
Produce a high quality and meaningful Life Plan with specific steps and resources needed to implement the plan for their future. A complete road map with specific recommendations with estimated costs. This will facilitate a bridge to planning with the client’s Wealth Manager.
Annual Check-In for 5 Years
Care Manager will perform a telephonic annual check-in with clients to inquire if they have had a status change that would require modifications to their plan. The degree of change in functional level, new medical diagnosis, or other factors will dictate the need for a plan revision.
Life Plan Revision and Symphony Care
At the time of check-in or at any time during the year, clients can access Care ManagementServices at the hourly rate. The full range of Care Management Services are offered along withrevisions to the Symphony Life Plan.
