A Vial of Life form gives emergency medical personel critical information about your medical history, current diagnosis and medications that allow them to care for you with the best outcome. In addition, they know who to contact first and who your trusted decision makers are if you are unable, even temporarily to make your own health care decisions.
Place this completed form in a well marked envelope that states,
“Emergency Information for Your Name”
Add into this emergency packet a copy of your Heath Care Proxy and any additional advance Directives.
Place the emergency packet on the back of your front door or on your refridgerator. This is where Paramedics will look for this vital information. Keep the Vial of Life Updated.
