When family members are struggling with cognitive and physical decline and live far away from their primary caregivers who are often their children, it is common that a relocation is needed. Often, this happens emergently with a sudden health crisis. At Symphony Care Management we are able to quickly respond to this time sensitive need. We will:

1)     Speak with you and your loved one to understand the goals for the client(s). We will gather the relevant history, financial information and begin building a trusting partnership with all parties.

2)     Coordinate with the hospital discharge planners to assess our client’s needs and appropriate discharge options. We will strongly advocate for our client(s) in the health care setting to prevent premature discharge.

3) If our clients are at home, arrange for an assessment in our client’s home so that the appropriate and optimal level of care and living options are identified.

4)     Make viable, quality and specific recommendations to family and the client of their options and assist them in implementing a plan that they agree meets their goals. Good information and understanding leads to good decision making and a move to the right environment.

Navigating the Transition

When necessary, we will collaborate with an Aging Life Care Professional in our network across the country who can assist with the assessment of an out of state client. We are also there to help you with health insurance changes if coming from out of state.

Finding the Right Setting

Once we have a good understanding of your loved one’s situation and needs, we will recommend the best quality skilled nursing facilities and assist in the admissions process. Our experience allows us to give you deep insight into the differences between various facitilities or assisted living communities and why specific ones might be a better fit than others for your loved ones needs.

Travel Arrangements

Utilizing our long-standing relationships with a number of facilities we will coordinate transitions and are available to assist with interstate transport which may include ambulance, chair car, med flight and possibly the need for a private caregiver to accompany the client.

Home from the Hospital or Rehabilitation Facility

For those clients transitioning to live with their family members, Symphony Care is committed to finding a quality primary care physician and other medical specialists as required for clients who are relocating. Prior to this move, we will assess the home environment for safety and recommend any home modifications needed. We will bring in the best vetted private care partners and coordinate a team of caregivers, layering in a variety of services that will benefit our client and their primary caregivers. Part of this process includes evaluating opportunities to access available entitlements. Care coordination between certified services and non-medical home care is key to a smooth transition for our clients and gets them on their path to recovery.

Throughout this stressful time, Symphony Care is there to support family members through our Family Caregiver Coaching and Alzheimer’s Coaching services.
